Add, update and delete exam questions over the API

Managing the questions in an exam one call at a time just got a lot less fiddly. POST /exams/{id}/questions now takes a question, or an array of up to 100, and returns the added questions as an array. Heads up: the body is the question itself now, so drop the old { "question": … } wrapper.

PATCH can now change the title, description, settings, tags, externalId, metadata and sectionId, and the new DELETE /exams/{id}/questions/{questionId} removes a question. Every question route also accepts your own externalId, or first and last, in place of {questionId}, so you don't need to keep Examplary's IDs around.
